Harper Terrace Dental (“we”, “us”, “our”), previously known as Law & Wong Dental, is a dental practice at Unit 15, 2 Harper Terrace, South Perth, WA 6151. We respect your privacy and handle your personal information, including your health information, in line with the Privacy Act 1988 (Cth) and the Australian Privacy Principles (APPs). This policy explains how we collect, use, disclose and protect personal information, including information collected through this website.
1. What information we collect
The information we collect depends on how you deal with us.
- Website enquiries. When you use our contact form we collect your name, phone number, email address and any message you choose to write.
- Calls, emails and visits. We keep a record of your contact details and what you ask us.
- If you become a patient. We collect identity and contact details, date of birth, emergency contacts, health fund and Medicare or concession card details where relevant, your medical and dental history, and records of your care, such as notes, x-rays, scans and photographs. Health information is sensitive information under the Privacy Act.
- Technical information. Like most websites, our hosting provider records technical data such as your IP address and the pages requested, for security and to keep the website running.
2. How we collect it
We collect personal information directly from you wherever we can: in person, by phone, by email, through our website form, or on forms you complete. Sometimes we receive information from other people, such as a referring practitioner, a parent or guardian, or your health fund, and from your authorised representative. Where we collect information about someone else, we expect you to have their permission to give it to us.
3. Why we collect and use it
- to respond to your enquiry and book appointments;
- to assess, plan and provide dental care, and keep accurate clinical records;
- to manage accounts, process payments and claim from health funds;
- to remind you about appointments and follow up on your care;
- to run our practice, including quality improvement, training and handling complaints;
- to meet our legal and professional obligations.
We will only use your information for another purpose if you consent, if it is related to the purpose we collected it for and you would reasonably expect it, or if the law allows or requires it.
4. Who we share it with
We do not sell your personal information. Depending on your situation, we may share it with:
- other health practitioners involved in your care, for example when we refer you to or work with a sleep physician, specialist or your doctor;
- dental laboratories and suppliers that make appliances, mouthguards, aligners or restorations;
- health funds, Medicare or other payers, and insurers, where needed for claims;
- professional advisers, regulators and courts, where required by law;
- service providers that help us run the practice, such as practice management software, IT support, website hosting and email providers, who must protect your information.
We only send you marketing messages if you have agreed to receive them, and you can opt out at any time.
5. Sending information overseas
Some of our service providers store or process data outside Australia. In particular, enquiries sent through our website form are protected against spam by Cloudflare, who also hosts this website, and are delivered to us by an email delivery provider, and those providers may handle data in the United States or other countries. We take reasonable steps to make sure overseas providers handle personal information in line with the APPs. To limit risk, please do not put detailed health information in the website form. Phone us instead.
6. Cookies and tracking
This website does not use advertising or analytics cookies. The contact page uses Cloudflare Turnstile to tell people from automated bots, which may use limited technical information from your browser and may set a technical cookie. Fonts and images are served from this website itself, so loading our pages does not send your details to other third parties apart from Cloudflare, which hosts the site.
7. Keeping your information safe
We store information in paper and electronic form and use reasonable safeguards against misuse, loss and unauthorised access, such as passwords, access controls and secure systems. We keep health records for as long as the law and Dental Board of Australia guidelines require, and then securely destroy or de-identify them. If a data breach is likely to cause serious harm, we will notify you and the Office of the Australian Information Commissioner as required by the Notifiable Data Breaches scheme.
8. Accessing and correcting your information
You can ask to see the personal information we hold about you and ask us to correct anything that is inaccurate, out of date or incomplete. Contact us using the details below. We may need to confirm your identity, and we will respond within a reasonable time, usually within 30 days. In limited cases the law allows us to refuse access, and if we do we will explain why.
9. Complaints
If you have a concern about how we have handled your information, please contact us first so we can try to fix it. If you are not satisfied with our response, you can contact the Office of the Australian Information Commissioner (oaic.gov.au) or the Health and Disability Services Complaints Office in Western Australia (hadsco.wa.gov.au).
